District 200 Hosting a Vaccine Clinic

On Saturday, May 22, District 200 is partnering with Walgreens to host a vaccine clinic at Pleasant Hill Elementary School located at 1N220 Pleasant Hill Road in Winfield. This vaccine clinic is available by appointment and appointments can be made by following this link. This clinic is open to anyone in the community eligible for the vaccine. 

Please make sure to fill out the form with all the applicable fields completed in advance and bring the printed form. A parent or guardian should be present for anyone under 18 years of age. If the parent will not be present, the parent MUST have filled out and SIGNED the form in advance. Anyone receiving the vaccine should wear clothing that will be easier for vaccine administration.
